Yes, you need your PUBLIC ip address to play with other people.
You just click "create network game" and click "ok". Than you will see an list of connected player. First entry should be your own nickname. Now you can tell the other players you want to invite your PUBLIC ip address. They have to click "join network game", put in your ip address and click "connect". If connection was successful established you will see the nick of the connected player in your list. If all players you want to play with are connected just click "start" and the game will do that.
It is important to share your PUBLIC ip address. If your are connected via "analog modem", "ISDN" or "DSL modem" the ip address of your computer is mostly the PUBLIC one.
If you are behind a router (e.g. WLAN-router) there is also a local network with local ip addresses. In this case your computer mostly have an local address like "192.168.0.4" or "192.168.1.65" or "192.168.2.3". Many default local addresses starts with "192.168.x.x". Please do not share your local address with your friends if you like to play over internet.
If you want to play PokerTH behind a router you have to setup port-forwarding for port 7234 (PokerTH default port) to your local ip address. On some routers these feature is also called "virtual servers". So if your local address is "192.168.2.2", you have to tell the router that every incoming request on port 7234 should be forwarded to 192.168.2.2 on port 7234. If you have to select UDP or TCP please choose TCP.
If the port-forwarding is set up you just have to look for the PUBLIC ip address of your router (called "WAN IP" e.g.). Please share THIS address with your friends.
Many routers can be set up via web-browers. To do this try to fill your browsers address field with a suggested local ip of your router. For example try "192.168.2.1" or "192.168.1.1" or "192.168.0.1". I attached a screenshot of my router setup via firefox. Another way to setup is to look in your routers "owner manual" and read how to do this
